I. Case Management By Licensed Professionals
Our client-focused program is designed to coordinate care services for your loved one regardless of the healthcare setting. We provide licensed professionals with experience in geriatric care who are experts in managing and providing care for the needs of the elderly and their families. Our goal is to help our clients gain independence and improve quality of life through exceptional care. These services are especially appropriate for seniors who live by themselves or for aging loved ones whose families do not live nearby.
How does it work?
A care manager will assess the needs of the client with family members and others involved in care. After identifying needs and sources of difficulty, we will design a personalized care plan and recommend appropriate services with consideration to cost effectiveness.
The client and family can choose to have a one-time consultation or decide to have the licensed care manager coordinate and monitor all aspects of care. If the family chooses ongoing care services through Boundless Care, the licensed care manager will manage everything and will be responsible for integrating all health services for the client—administering personal care services, coordinating medical appointments, arranging transportation and much more—to maximize and provide the best quality care your family needs and deserves.
II. Private Duty Nursing / Short and Long Term Care By Professional Registered Nurses
Our nurses are licensed, knowledgeable and trustworthy, and are qualified to take care of the ailing members of your family. They can work short or extended hours (minimum of four hours) depending on your needs.
Registered Nurses To Visit You
Friendly, caring and knowledgeable nurses visit twice a week to monitor the health of your loved ones. During their visits they monitor vital signs, perform physical assessments, review medications, check medical appointments, review eating habits/nutritional status, administer range of motion exercises, and provide other services the client needs most. We assure you, the care our employees bring is something your loved ones can look forward to!
III. Licensed Independent Nurses for Organizations / Companies
Our sub-contractor RNs (Registered Nurses) are licensed and insured and are contracted based on stringent qualifications. They have many years of experience in nursing care. They are trained and skilled in performing procedures and specific orders such as wound care, Foley catheter care, blood glucose monitoring, insulin administration, IV therapy, blood drawing, and other procedures within the scope of nursing practice.
IV. Support Care Services
Our experienced Caregivers, Licensed Health Aides, and Certified Nurse Assistants provide assistance with the activities involved in daily life, including
- BATHING
- MOBILITY TRANSFERS and POSITIONING PROGRAM
- INCONTINENCE CARE, TOILETING
- FEEDING and MEAL PREPARATION
- LIGHT HOUSE WORK
- ASSIST or REMIND CLIENTS CONCERNING TIMELY MEDICATIONS and other personal care needs

Our companionship services provide caring staff that can simply spend quality time with your loved ones. Transportation Services
Worried about difficulties driving family members to physicians or primary healthcare centers or about taking them visiting or to other appointments? We have contracted the services of Boundless Transport, LLC, a non-medical transport service that can drive clients wherever they want to go—medical and dental appointments, shopping, seeing friends, or simply for a joy ride!
