Our client-focused program is designed to coordinate care services for your loved one regardless of the healthcare setting.  We provide licensed professionals with experience in geriatric care who are experts in managing and providing care for the needs of the elderly and their families. Our goal is to help our clients gain independence and improve quality of life through exceptional care.  These services are especially appropriate for seniors who live by themselves or for aging loved ones whose families do not live nearby.

How does it work?

A care manager will assess the needs of the client with family members and others involved in care. After identifying needs and sources of difficulty, we will design a personalized care plan and recommend appropriate services with consideration to cost effectiveness.

The client and family can choose to have a one-time consultation or decide to have the licensed care manager coordinate and monitor all aspects of care. If the family chooses ongoing care services through Boundless Care, the licensed care manager will manage everything and will be responsible for integrating all health services for the client—administering personal care services, coordinating medical appointments, arranging transportation and much more—to maximize and provide the best quality care your family needs and deserves.
